Registered Nurse Care Manager

A company is looking for a Nurse Care Manager to manage patients' specialized needs and coordinate care for individuals with complex health conditions.

Key Responsibilities

Manage patients with complex chronic conditions and develop comprehensive care plans

Coordinate care delivery among healthcare providers and monitor patient outcomes

Educate and advocate for patients to empower them in managing their health conditions

Required Qualifications

Current Registered Nurse license in Michigan and / or New York

Bachelor's of Science in Nursing preferred

3+ years of clinical practice in a healthcare setting preferred

Case management experience with a senior population preferred

Prior experience with electronic health records required
