A company is looking for a Care Manager Transition of Care to assess, plan, and coordinate medical services for post-discharge members.Key ResponsibilitiesEvaluate member needs through post-discharge assessments and medication reconciliationDevelop care / service plans and collaborate with interdisciplinary teams to support member transitionsFacilitate services between medical providers and non-medical resources to meet member needsRequired Qualifications, Training, and EducationMaster's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing2 - 4 years of related experienceLicensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
