CORE Community Organized Relief EffortAtlanta, GA, United States- Full-time
CORE (Community Organized Relief Effort) is a nonprofit organization whose mission is to build sustained, community-based response programs to natural disasters. In response to the COVID-19 pandemic, CORE GA provides free vaccinations and additional resources to disadvantaged communities and emerging hotspots around Georgia. This is accomplished through maintaining strategic partnerships with local health agencies and community-based organizations to reach high-risk populations in vulnerable areas. The Health Outreach Coordinator will be responsible for outreach and education to organizations and individuals throughout the communities served. Additionally, the Health Outreach Coordinator will lead out on benefits navigation and resource coordination activities.
Responsibilities :
- Understand and effectively communicate CORE's mission, vision, core values & strategic direction to external groups
- Conduct intake, follow up, refer, and assist clients to community resources and public assistance programs.
- Assist clients with screenings and applications to public assistance programs such as SNAP, WIC, TANF, CAPS, Medicaid, etc.
- Develop trust and rapport with clients and refer to additional programs and resources as needed
- Maintain and share community resources information with CORE GA program staff and clients
- Track and monitor program data and deliverables
- Identify and develop relationships / partnerships with resource organizations in designated areas who can participate in / lead response activities
- Engage with formal and informal community leaders and influencers, including churches, local businesses, neighborhood associations, and other civic organizations
- Work closely with CORE GA field operations teams to provide a continuum of services
- Adhere to all program guidelines, policies, protocols and workflows
- Report to Area Manager for direction and communicate significant incidents
- Other duties as assigned

25 - $25 an hour
In accordance with law, the expected pay range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
