  • POSITION SUMMARY :

    A Supports Coordinator provides family-focused services to children with developmental disabilities for the purpose of assisting the child in building on their strengths and helping the family to remain safely together in the family home. The Supports Coordinator will work in an environment convenient to the child and family to assist with linking, monitoring, and coordinating services.

    ESSENTIAL RESPONSIBILITIES :

  • To provide support and resources to families and children diagnosed with developmental disabilities.
  • Able to meet clients and their families in non-traditional settings, (i.e. client’s home or public place that can accommodate privacy) during hours that meet the family’s needs.
  • Participate in at least bi-weekly supervision at minimum and participate in bi-weekly team meetings.
  • To effectively coordinate, educate and train family members and support systems on individual needs of each child.
  • Demonstrate self-care strategies and model healthy coping mechanisms when exposed to emotionally challenging circumstances
  • To monitor individuals for their progress on goals as well as to provide additional support and / or resources.
  • Demonstrate sensitivity to cultural, ethnic and linguistic differences in all interactions with youth and families.
  • Work skillfully and tactfully with other people significant to the client including families, schools, doctors, and other systems within the client’s life.
  • Support families in navigating systems that may not reflect their cultural values, advocating for culturally responsive care.
  • Acts as a cultural bridge between youth, families and community groups from different backgrounds.
  • Maintain program compliance in timeliness of services and required paperwork completion.
  • Provide crisis intervention and on-call response services as assigned by supervisor.
  • Maintain coordination for treatment planning with multiple services providers.
  • Supervision Received : Specific Direction - Follows established work procedures. Receives periodic checks for performance. Refers unique questions / conditions to immediate supervisor.

    Supervisory Responsibilities : No supervisory responsibilities.

    MINIMUM QUALIFICATIONS :

  • Minimum education requirements include a 4-year degree in a human services field.
  • One year’s experience working with people who have been diagnosed with a developmental disability preferred.
  • Possess a knowledge of community resources and support for children and families, and the skills necessary to facilitate and encourage positive change towards treatment goals.
  • Valid driver's license, good driving record and adequate insurance with a car available always.
  • Basic computer skills with knowledge of Microsoft Outlook, electronic record systems and other software programs.
  • The ability to articulate ideas and prepare written reports and correspondence that are of a high professional standard, are grammatically correct and are timely.
  • Ability to collaborate with school, department of health & human service, behavioral health and medical health personnel.
  • Maintain knowledge of and follow the code of ethics for your profession and agree to abide by the related agency policies.
  • Ability to be self-directed and autonomous while also operating as an interdisciplinary.
  • The ability to work with a variety of clients and staff of differing socio-economic, cultural, and ethnic backgrounds.
  • Personal qualities of warmth, sensitivity, and ability to establish meaningful therapeutic relationships.
  • Personal qualities of organizational skills, assertiveness, good judgment, conscientiousness, attention to detail, and timeliness.
