Dental Hygienist
Tapestry 360 Health, a Federally Qualified Health Center (FQHC), is seeking a highly skilled and compassionate Dental Hygienist to join our team. This position plays a critical role in providing high-quality oral health care and education to a diverse patient population. The ideal candidate will have a strong commitment to promoting preventive care and contributing to the overall mission of increasing access to healthcare in underserved communities.
- Provide direct patient care and dental services as trained, licensed and assigned. Assess dental condition and needs of patient using approved patient screening procedures, including medical history review, dental charting and periodontal charting.
- Document dental history and chief complaint; record patient vital signs and any other dental observation as clinically appropriate and required by policy.
- Document lab procedures and ensure follow up on results.
- Follow through with oral hygiene procedures in accordance with treatment plans prescribed the supervising dentist.

Required :
Basic Life Support (BLS) certification.
Current and valid Registered Dental Hygienist (RDH) license in the state of Illinois.
Preferred : Public Health Dental Hygienist (PHDH) certification. Candidates who do not hold the PHDH certification must have a strong desire to pursue it.
