The Care Manager plays a pivotal role in coordinating and managing comprehensive care plans for individuals requiring long-term support within the healthcare and social assistance sector. This position focuses on assessing member's needs, developing personalized discharge plans, and ensuring seamless transitions between care settings to promote optimal health outcomes. The Care Manager collaborates closely with multidisciplinary teams, including healthcare providers, social workers, and community resources, to facilitate access to Medicaid and other support services. By advocating for members and monitoring their progress, the Care Manager ensures that care delivery aligns with both clinical guidelines and individual preferences.
