Job descriptionUnder the physician's order, admit patients eligible for hospice services Assess and evaluates patient needs / problems as outlined in the patient IDG Plan of Care; identify mutually agreed upon goals with patients and families Document daily changes and updates to the patient care plans, including discharge planning, and notify attending and hospice physician through scheduled IDG meetings Participate in Quality Assessment and Performance Improvement (QAPI) program through submission of data collection relating to patient care problems Attend skilled nurse in-services and meetings, and interdisciplinary team meetings Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care Current unencumbered Registered Nurse (RN) state license or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC) Three years of experience as a registered nurse in a licensed hospice or home health setting including team supervision Current driver's license and automobile in good working condition with proof of auto insurance Demonstrates above average computer and EHR competency Able to handle multiple and shifting assignments Ability to make sound professional clinical judgment in a field setting
