A direct hire opportunity is available for an experienced Oncology / Hematology Outpatient Pharmacist to join a well-established medical center in Cicero, IL. This role focuses on providing expert clinical pharmaceutical care within a hospital outpatient setting, supporting oncology patients through comprehensive medication management and collaboration with the multidisciplinary healthcare team.

Key Responsibilities :

• Participate in interdisciplinary rounds to assist with the design, implementation, and monitoring of medication regimens for malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant patients.
• Review and assess drug therapies for appropriateness, dosage, interactions, and cost-effectiveness.
• Reconcile home medications upon patient admission.
• Communicate with prescribers to clarify medication orders and document changes thoroughly.
• Provide patient and family counseling on medication regimens, adherence, and adverse effects.
• Perform pharmacokinetic consultations and document kinetic notes for specific medication dosing.
• Monitor, document, and report adverse drug reactions and medication errors.
• Review chemotherapy orders accurately and efficiently.
• Support nursing staff by addressing medication-related inquiries.
• Participate in evening staffing as required.
