Under the general direction of the Clinic Operations Manager, the Patient Outreach Specialist will provide patients / individuals with the education needed about our services, assist with the enrollment process, support new membership projects, retention assistance, and develop relationships with strategic community partners.
Job Responsibilities / Duties
- Enters new patient demographics and updates established patient information, as necessary, in EHR to maintain accurate patient medical records.
- Obtains both new and established patient insurance information including eligibility and authorization necessary.
- Assist patients in applying and / or aligning their health care coverage plans, providing necessary follow-up, and data collection.
- Assisting patients identified to navigate the enrollment process to secure needed services. This includes working on new additions and loss of patients in various networks.
- Outreach and schedule patient appointments as necessary to ensure membership retention and growth for the group. This includes phone calls, email outreach as well as letter and mail-out preparation.
- Outreach and educate patients regarding how and where to access health care services. Outreach to members of new services and new providers.
- Assists in conducting assessments of the retention efforts and identifying areas of improvement.
- Demonstrates flexibility with work schedule and work assignments, including floating to other medical offices.
- Promptly answers and responds to all calls in a professional manner. Acts as a liaison between patients, referring and treating physicians, staff, hospitals, and others involved in patient care.
- Identifies patients’ important needs, answers to their questions and assists with informed patient decisions that may arise prior to and / or post office visits appointment time.
- Provides patients with practical resources and offers emotional support and understanding throughout the patient care coordination process.
- Outreach to patient membership that have overutilized hospital services.
- Maintain quality customer satisfaction.
- Monitors patient requests and outgoing messages for various patient experience initiatives such as Follow My Health.
- Assists with front office duties where necessary to assist patients to uphold patient experience and support operations including but not limited to clinical setting deliverables.
- Prepares welcome materials for new patients and current patients.
- Participates in planning discussions and meetings. Assists in developing tactical plans to support outreach and patient membership goals. Prepare and submits weekly productivity reports.
- Other duties as assigned.
Qualifications
Minimum Education : High School Diploma required. College Courses and / or associate degree in healthcare administration preferred.
Minimum Experience : 3 - 5 years of health care experience in customer service, administration and / or patient care coordination required.
Specific Requirements :
Bilingual in Spanish a must (speaking, writing)
