Job descriptionAnswer and triage incoming calls related to appointments, medications, and new patient inquiries Coordinate with pharmacies for medication verifications, refills, and prior authorizations Support patients with accessing telehealth platforms and troubleshooting basic technology issues Facilitate communication and record-sharing with external agencies and providers Participate in care coordination, discharge planning, and treatment plan implementation Perform and document vital signs : temperature, pulse, respiration, blood pressure, weight, and urinalysis Order labs and imaging and manage results under provider supervision Verify insurance coverage and calculate co-pays Prepare patient letters and forms (e.g., disability, ESA documentation) Onboard new patients following established protocols Prepare and track referrals; fax and upload medical records as needed Schedule meetings with community agencies and pharmaceutical representatives Oversee medical supply inventory, ordering, and ensure safe storage and disposal Document communications and tasks thoroughly in the electronic health record (EHR) and patient portal

The Care Coordinator plays a key role in ensuring exceptional patient experiences and smooth daily operations within our clinic. This in-person position supports both clinical and administrative functions across our four locations, with a primary on-site presence in Pueblo and occasional coverage at our Colorado Springs offices. As part of a small, dedicated team, the Care Coordinator must be flexible, reliable, and detail-oriented.
Primary Responsibilities :
Reception & Patient Support
- Greet and check in all in-person patients
- Collect and update patient information for both in-person and telehealth visits
- Provide information, education, and assistance regarding services and care
- Assist patients with completing forms and accessing the online portal
- Collect and review drug test results; consult with providers as needed
Communication & Coordination
Clinical & Administrative Duties
