Responsibilities
- MAJOR RESPONSIBILITIES
- 1.Patient Support :
- a.Will receive the patients from Patient Development department through a warm handoff process;
- b.Attend the patients' doctor visits to provide support, address concerns, and facilitate communication between patients and healthcare providers;
- c.Conduct home visits to assess patients' needs, deliver resources, and provide additional assistance as required;
- d.Conduct an initial set up home visit for patients newly approved for home health care. Provide information on the program, as well as facilitate caregiver onboarding;
- 2.Resource Delivery :
- a.Deliver comprehensive patient binders to patients' homes containing plan of care and training information;
- 3. Communication :
- a.Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care;
- b.Make courtesy calls to patients to check on their well-being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team;
- c.Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care;
- 4. Advocacy :
- a.Act as a patient advocate by collaborating with healthcare providers to address patient concerns and ensure patients receive high-quality, personalized care;
- b.Advocate for patients' needs and rights within the organization, promoting a patient-centered approach to healthcare delivery
