Job Description

Summary :

The Medical Social Worker (MSW) provides comprehensive psychosocial support to patients and families within an acute hospital environment. This role focuses on discharge planning crisis intervention and coordination of community resources to ensure continuity of care post-discharge. The MSW collaborates with the interdisciplinary care team to assess psychosocial needs advocate for patients and develop care plans that align with clinical and social determinants of health.

Responsibilities :

Conduct biopsychosocial assessments and develop individualized care and discharge plans.

Facilitate crisis intervention counseling and support for patients and families facing emotional social and financial challenges.

Coordinate discharge planning and arrange post-acute care services including home health rehabilitation hospice and community resources.

Collaborate with nursing physicians and case managers to ensure timely transitions and optimal outcomes.

Provide education to patients and caregivers regarding available community resources (Medicaid Medicare Social Security TANF housing and medication assistance).

Document all patient interactions assessments and care coordination activities in Epic per regulatory and hospital standards.

Participate in interdisciplinary rounds and case conferences to address complex psychosocial and discharge needs.

Assist with referrals to Adult / Child Protective Services Guardianship and Veterans programs as appropriate.

Advocate for patients rights and ensure compliance with NASW Code of Ethics and hospital policies.

Manage a caseload of approximately 20 23 patients in an acute inpatient setting.

May float to nearby Laguna facility as needed.

Key Requirements :

Minimum 2 years of experience as a Medical Social Worker in an acute hospital setting .

Must have at least 1 full year of recent acute care experience -redirect candidates who do not meet this.

No first-time travelers (hard stop).

Must be proficient in Epic EMR.

Must be comfortable with high patient caseloads (20 23 patients).

Weekend Requirement : Every other weekend; must also be available for holidays.

CA State License required

RTO Restriction : No more than 7 days per contract.

Floating : May float to Laguna facility.

Travel Experience : Required.

Qualifications

Education :

Masters Degree in Social Work (MSW) from an accredited program.

Experience :

Minimum 2 years of inpatient acute care hospital experience as an MSW.

Prior experience in discharge planning case management and crisis intervention required.

Experience working with multidisciplinary teams in trauma ICU NICU or medical / surgical units preferred.

License :

California LCSW preferred; MSW required.

BLS certification required.

Pending CA licensure

Skills :

Clinical & Case Management : Biopsychosocial assessment discharge planning needs assessment plan of care development utilization of DME and community services.

Regulatory Knowledge : CMS HIPAA ADA NASW Code of Ethics and state-specific hospital standards.

Crisis Intervention & Counseling : Skilled in managing high-risk situations (suicide abuse substance use trauma).

Community Resource Coordination : Medicaid Medicare Social Security TANF AFDC housing and Veterans programs.

Documentation : Thorough EMR charting discharge documentation and patient / caregiver education tracking.

Preferred Exposure : Trauma Level I / II hospitals Behavioral Health ICU Pediatrics NICU and Emergency Department.
