Job Description
Job Summary : The Housing Coordinator is responsible for facilitating the application process and transition clients into affordable housing within the community. Additionally, this role provides continuous support services to ensure sustained housing stability for clients.
Responsibilities : - Assist clients in identifying and securing appropriate housing options within the community.
- Develop and implement individualized, person-centered housing plans.
- Connect clients with essential community resources and services.
- Conduct outreach to community organizations and relevant agencies to promote housing opportunities.
- Maintain regular communication with clients and their support networks to review progress, address issues, and plan future actions.
- Accompany clients on visits to potential housing sites.
- Aid clients in the completion of housing applications.
- Provide support for packing and coordinating moves.
- Offer follow-up services post-move to ensure ongoing housing stability
- Advocate on behalf of clients with community resources to prevent eviction and ensure safety.
- Identify and manage behaviors that could jeopardize continued housing.
- Provide in-service transportation as necessary.
- Collaborate with public benefit programs, employment opportunities, and landlords to promote housing retention.
- Maintain detailed and accurate progress notes as required.
- Participate in weekly team meetings and individual meetings as mandated.
- Ability to navigate Minnesota Healthcare programs and processes, maintain files and program records, and comply with HIPAA and Data Privacy Practices.
Requirements and Abilities : - Completion of Housing and Community Development (HAC DHS) training within the first week of employment.
