Experience : Minimum 5 years acute inpatient case management experience
License :
Registered Nurse (RN) with a valid, unrestricted state license
OR
Licensed Social Worker (LMSW) with a valid, unrestricted state license
Certifications : MLSW certification
BLS certification
Must-Have :
Experience in acute discharge planning
Previous experience using CarePort system
Ability to acclimate quickly in clinical settings
Description :
The Integrated Case Manager for Population Health is an essential member of the patient-centered care team.
This role involves assessment, planning, facilitation, care coordination, evaluation, and advocacy for patients and their families.
The focus is on addressing comprehensive health care needs through effective communication and available resources.
The position requires managing patients who have undergone a critical event or diagnosis demanding complex management and extensive resources.
Services are provided across various care settings including ambulatory, inpatient, and health plans.
Key Responsibilities :
Conduct comprehensive assessments of patient and family caregiver needs including biomedical, psychological, social, and functional factors.
Develop personalized, patient-centered care plans to optimize the care experience.
Engage patients and families in the care process through ongoing communication and education.
Utilize professional judgment and critical thinking to assist patients in overcoming barriers to meet their goals.
Provide counseling related to treatment decisions and end-of-life issues, including Advanced Care Planning.
Ensure seamless and safe transitions for patients between care settings.
Advocate for appropriate service delivery within the patient's health plan benefit structure.
Collaborate with care team members to manage patients with complex medical and social needs.
Evaluate patient responses to care plans and modify as necessary.
Participate in process improvement activities to enhance safety and quality of care.
Facilitate interventions in cases involving abuse or neglect.
Support departmental goals that contribute to organizational success.
Perform other duties as assigned.

Experience in discharge planning and community health is preferred.
