Our client is seeking a Licensed Graduate Social Worker - LGSW to work with older adults in a Washington, DC long term care facility. The schedule is 8 : 30am-5pm, Monday-Friday.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

• Conduct intake / assessment to determine needed services
• Meet with clients as determined by treatment plan
• Complete case notes in a timely fashion
• Work with the Director and other multi-disciplinary team members to develop and implement treatment plans / assessments for clients
