Behavioral Health Clinical Coordinator

Make an impact in Norristown, Conshohocken, and Bryn Mawr, Pennsylvania, as a Behavioral Health Clinical Coordinator within Montgomery County Residentials, a program within the nationally recognized Resources for Human Development (RHD) organization!

We have two full-time positions available that will be working Monday-Friday :   9 am-5 pm, between Norristown and Bryn Mawr and Norristown and Conshohocken. This role pays $22.20 per Hour.

Position Summary :

As a Behavioral Health Clinical Coordinator, you are responsible for organizing patient care activities and sharing information among all the participants concerned with a patient’s care to achieve safer and more effective treatment outcomes. In addition, the Behavioral Health Clinical Coordinator will oversee the supervision of support staff, including Care Coordinators and Resource Coordinators. A commitment to the RHD values should be demonstrated as job duties are performed.

Essential Duties and Functions :

Program Development and Monitoring :

• Maintain up-to-date knowledge and implementation of RHD programs.
• Be familiar with all program-related regulations from funders, community support services, government licensing agencies, and other related resources.
• Build and enhance relationships with RHD program staff and outside providers to coordinate care.
• Maintain a tracking system to identify the need for comprehensive treatment plans.
• Interface with in-house and external partner providers through monthly / quarterly off-site visits.
