Pharmacy Director-Patient Care Management
The Pharmacy Director-Patient Care Management (PCM) contributes to AHF by directing the operations of the Patient Care Management Hub (PCMH), ensuring delivery of high-quality, patient-centered medication management that optimizes outcomes and maximizes P4H revenue capture across our network of pharmacies.
The Pharmacy Director-PCM enhances the reputation of AHF by overseeing the transfer, adjudication, and maintenance of prescriptions from home pharmacies to centralized hubs. This role ensures that each patient receives consistent, compliant, and compassionate carewherever they are servedwhile reinforcing AHF's mission to provide cutting-edge medicine and advocacy for all.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The Pharmacy Director-PCM will work as a part of a close-knit team to lead the centralization of refill processing from home pharmacies into the Patient Care Management Hub (PCMH), overseeing pharmacists and technicians responsible for medication reconciliation and workflow optimization. Through strong collaboration with Regional Pharmacy Directors, Pharmacy Managers, and Clinical Operations, the PCM Director ensures compliance with all state and federal standards, maintains financial integrity, and promotes innovation through centralized prescription processing and patient-centered medication management.
Licenses and Certifications : State license for the location you are applying for, one additional State license. Will be required to obtain AAHIVP Certification within one year of hire.
Schedule : Monday through Friday, 8 : 00 AM to 5 : 30 PM, with occasional weekend coverage as business needs increase.
