Pay Range : $24 - $25 / hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities :

• Shift Timings : 9 AM - 6 PM EST Mon to Fri
• Conduct outbound calls to members to discuss and follow up on care / service plans
• Coordinate care activities with healthcare providers, community partners, and members / caregivers
• Assist with member inquiries, concerns, and service-related requests
• Perform service assessments / screenings and document member needs as required
• Maintain member records in compliance with state and regulatory requirements
• Identify needs and make referrals to care managers, disease managers, or community resources
• Provide education on benefits and resources available to members
• Complete daily production reports and meet call volume expectations (50–75 calls per day)
• Ensure professional conduct in a remote work environment, with the ability to work on-site if needed (Tampa office location available).
