Job description5+ years of experience : $25,000 Full Health Benefits Reimbursement for Mileage and Toll Fees Focus : Post-discharge visits, including : Social service follow-ups General follow-up care and care coordination Caseload : 6-7 patients per day Support : Medical Director available for guidance Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate National certification in at least one of the following specialties : Adult-Gerontology Primary Care Nurse Practitioner Adult Nurse Practitioner Gerontological Nurse Practitioner Active, non-probationary state Nurse Practitioner license Active DEA license US work authorization

Salary & Benefits :
- Salary Range : $105,000 - $135,000 (Experienced providers with bonuses can earn $150,000 - $160,000)
- Annual Clinical Performance Bonus :
Less than 5 years of experience : $15,000
