Join to apply for the Clinical Rehab Specialist PRN role at St. David's HealthCare . We are seeking a Clinical Rehab Specialist to support high-quality, efficient care at St. David's Rehabilitation Hospital.
This role focuses on coordinating between internal and external referral sources and acute inpatient rehab operations to ensure appropriate patient placement and seamless admission processes.
Responsibilities
- Act as a coordinator between internal and external referral sources and acute inpatient rehab operations.
- Establish relationships with case managers, payors, and other referral sources.
- Ensure appropriate patient placement through clinical screenings and assessments in collaboration with hospital case management and the rehab Medical Director.
- Identify patients who may need acute inpatient rehabilitation services and pursue formal pre-admission assessments as directed by the case manager.
- Assist with and coordinate the intake and pre-admission screening process.
- Document evidence of the patient's compliance with rehabilitation admission criteria on pre-admission forms in a timely and legible manner.
- Provide information to the Rehab Medical Director and respond to requests for additional information to support admission decisions.
- Conduct on-site and off-site reviews to identify patients who could benefit from rehab services.
- Perform clinical evaluations to determine appropriateness of admission for rehab services.
- Provide information about inpatient rehab and program features to Case Management to share with patients and families.
- Explain participation requirements to patients and families upon admission and offer tours of the rehab unit.
- Secure information related to patient resources and benefits.
- Demonstrate knowledge of rehab admission criteria and advise referral sources on inpatient rehab criteria.
- Establish and maintain effective working relationships with referral sources.
- Complete face-to-face educational contacts with physicians, case managers, and other professionals; serve as an educational resource to patients, families and caregivers.
- Participate in market analysis, data analysis and development of business plans for the rehab unit; contribute to referral source materials and outreach tools.
- Assist in developing the annual strategic plan for the rehab unit / hospital, including community outreach to raise awareness of services.
- Ensure referral sources receive timely follow-up communications regarding patients and hospital programs.
- Develop a system to ensure calls are handled 24 / 7.
Qualifications
Experience
Minimum of 2 years of recent clinical care experienceExperience in an acute or inpatient settingEducation
Graduate of an accredited school in clinical specialty (Associate's degree, Bachelor's degree, Master's degree or Doctorate degree)Licenses and Certifications
Licensed professional in a clinical healthcare specialty (RN, Occupational Therapist, Physical Therapist, Speech Therapist)Current state licensure in clinical practice areaBasic Life Support certificationAbout the Organization
