Job descriptionPatient Care : Conduct comprehensive assessments and provide skilled nursing care to patients in their homes, ensuring care plans are executed with precision. OASIS Documentation : Complete OASIS (Outcome and Assessment Information Set) documentation accurately to ensure proper patient care planning and compliance with regulatory standards. Homecare Homebase : Utilize Homecare Homebase (HCHB) software to document patient visits, manage care plans, and communicate with the healthcare team. Coordination : Collaborate with physicians, physical therapists, and other healthcare professionals to deliver coordinated, patient-centered care. On-Call Support : Provide weekend on-call support on a rotational basis, addressing urgent patient needs. Patient Education : Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health. Active New YorkRegistered Nurse (RN)License Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross Minimum of 1 year of experience as an RN in a home health setting Proficiency in OASIS and Homecare Homebase (HCHB) software required Oasis Certified strongly preferred Reliable transportation and a valid drivers license Ability to complete Joint Commission onboarding standards

In this role, you'll deliver compassionate, top-tier care to adult and geriatric patients, helping them maintain their health and independence in the comfort of their own homes.
Position Details :
- Hiring Company : Sunbelt Staffing
- Client : Nationwide Home Health Agency
- Location : Hawthorne, NY, and surrounding areas
- Schedule : Full-time, 40 hours per week (Monday-Friday, 8 : 00 AM - 5 : 00 PM)
- On-Call : Typically, one weekend per month
- Patient Population : Adult and Geriatric
- Productivity : 30-32units per week, up to 35 visits per week
- Mileage Reimbursement : $0.48 per mile (visit to visit), 150-250 miles per week expected
- Benefits : Medical, Dental, Vision, 401(k) after 90 days
- Pay : Negotiable based on territory budget and candidate experience; paid by hour - not by visit; blended pay packages available for travelers
