We are seeking a compassionate and skilled Advanced Practice Provider (APP) to deliver comprehensive healthcare services to patients across their care journey. This role supports patient outcomes, continuity of care, and care in home and community-based settings such as assisted living facilities, skilled nursing facilities, group homes, and other delegated locations.
Base pay range
$57.00 / hr - $60.00 / hr
Key Responsibilities
- Conduct a minimum of 25 comprehensive in-home or community-based patient visits per week.
- Perform thorough assessments including medical histories, physical exams, risk assessments, and medication management.
- Diagnose and develop evidence-based treatment plans.
- Manage chronic conditions (e.g., diabetes, hypertension, cardiovascular diseases) and educate patients on disease control.
- Prescribe, adjust, and educate on medications as needed.
- Provide preventive care such as immunizations, screenings, and lifestyle counseling.
- Empower patients through education on disease management and self-care.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless, coordinated care.
- Serve as the primary care provider of record for assigned patients.
- Maintain accurate EMR documentation within 48 hours of visits.
- Demonstrate exceptional bedside manner, building trust and rapport with patients and families.
- Conduct follow-ups and adjust treatment plans as needed.
- Participate in quality improvement initiatives and adhere to healthcare standards and regulations.
- Participate in on-call rotation (every 6–7 weeks) to provide after-hours support and visits (with additional compensation).
- Travel reliably within assigned territory for patient appointments.
- Comply with all company policies, compliance programs, and federal / state regulations on privacy and confidentiality.
