Location : Fountain Hills, AZ
Schedule : Full Time (Mon-Fri 0800-1700)
Wage : $200,000 - $250,000 / year base (plus bonus structure)
Benefits : Full Benefit Package
Call : No Call
Setting : Outpatient Family Medicine / Primary Care Clinic
Description :
Our company is currently seeking a Primary Care Physician to join our team. Our clinics focus on primary care and work with a group of specialist to provide complete care for all of our patients.
We are focusing on quality care and provide a workplace which our health team enjoy coming to work each day for patient care.

Additionally all candidates must meet all criteria listed below :
Responsibilities
- Well visits : Well Woman Exams, Complete Annual Physicals, AWV
- Same day appointments (fever, UTI, headache, allergies, etc)
- Chronic disease management (diabetes, hypertension, COPD, etc.)
- Preventive care, healthy living, disease prevention
- Ordering Diagnostic Testing & Labs as Needed
- Ability to Understand and Work with Specified Care Flows of Treatment
- Ability to use EHR System Proficiently & Complete Notes in a Timely Manner
Requirements :
- AZ Licensed Medical Provider (MD / DO)
- DEA License (AZ) Without Restrictions
- No History of Malpractice
- No restrictions with Credentialing with Medicare / medicaid / Commercial Insurances
