Job description

Healthcare Case Manager – Bilingual (English / Spanish) – North Miami Area

Location : Work From Home + Local Travel in Miami-Dade County (FL)

Territory : North Miami, Aventura, Hialeah, Miramar (must reside in this area)

Schedule : Monday–Friday, 8 : 00 AM–5 : 00 PM

Type : Full-time contract

Travel : 50–75% within Miami-Dade County

Pay Rate : $34.13 / hour

Overview

A-Line Staffing is seeking a Healthcare Case Manager – Bilingual (English / Spanish) to support members in the North Miami area. This role combines work-from-home flexibility with frequent field visits to members’ homes, assisted living facilities, and nursing homes.

You’ll assess members’ needs, coordinate services, and manage both supportive and medically complex cases to improve overall wellness and outcomes.

Key Responsibilities

  • Conduct telephonic and face-to-face assessments with members to evaluate medical, social, and environmental needs.
  • Develop and implement individualized case management plans , focusing on both short- and long-term health outcomes.
  • Coordinate services such as :

Prior authorizations

  • Communication with PCPs and skilled providers
  • Condition management information
  • Medication review
  • Community resources and supports
  • Manage a caseload of supportive and medically complex members , tracking progress and updating care plans as needed.
  • Identify opportunities to improve quality and compliance and follow through with appropriate actions.
  • Educate and empower members and caregivers to improve adherence, self-management, and overall wellness .
  • Use Microsoft Office (including Excel) and other systems to document activities, track outcomes, and report results.
  • Work Environment & Schedule

  • Work from Home for calls, documentation, and virtual meetings.
  • Travel 50–75% within Miami-Dade County for in-person member visits (homes, assisted living facilities, nursing homes).
  • Training : Conducted remotely via Microsoft Teams.
  • Hours : Monday–Friday, 8 : 00 AM–5 : 00 PM.
  • Mileage reimbursement to and from member homes per client policy.
  • Required Qualifications

  • Bilingual (English / Spanish) REQUIRED – fluent speaking and writing.
  • Residence : Must live in North Miami Dade County (North Miami, Aventura, Hialeah, or Miramar).
  • Education :
  • Bachelor’s degree required (nursing / social work not required; related fields welcome).

  • Experience :
  • Case management experience required.

  • Long-term care experience preferred .
  • Technical Skills :
  • Proficient with Microsoft Office , especially Excel .

  • Comfortable with virtual platforms (e.g., Microsoft Teams) and electronic documentation.
  • Core Competencies

  • Strong critical thinking and problem-solving skills .
  • Excellent customer service and communication (phone and written).
  • Highly organized, detail-oriented, and tech-savvy .
  • Able to multitask and manage a field-based caseload efficiently.
  • Comfortable in a patient-facing role and entering member homes and facilities.

  • Role is patient-facing and considered safety sensitive .
  • Must meet all client-specific background and compliance requirements .
