MDS Coordinator RN

Role Overview :

We're seeking a detail-oriented RN with MDS expertise to join our clinical team as an MDS Coordinator. You'll lead assessment processes, guide our MDS team, and play a critical role in ensuring accurate reimbursement and high-quality resident care.

Key Responsibilities :

Oversee and ensure timely, accurate MDS 3.0 assessments compliant with NYS and CMS guidelines

Lead MDS assessor team, coordinate care plan meetings, attend morning huddles, and drive utilization review

Ensure accurate documentation, attention to diagnosis coding, certifications, and Advance Beneficiary Notices in EMR

Use data to inform care strategies, support interdisciplinary teams, and coach staff on documentation accuracy

Qualifications :

RN license in New York State

Minimum 1–2 years of MDS 3.0 experience in SNF or post-acute care settings

Proficient with EMR systems and data-driven care initiatives

Monday–Friday, core business hours availability
