Case Manager Job Specifics
As a case manager you are to take care of your clients and caregivers assigned to you. In the event another manager is out of the office, we work as a team player to take care of all caregivers and clients. Schedules are subject to change. Part of case manager duties include:
· Meet and Greets
· Evaluations 30-60-90 days
· Scheduling your caregivers and clients assigned to you
· Supervisory visits (when going into homes to clean binders you are to fill out a supervisory visit form.)
· Office hours are Monday-Friday, rotating weekends
· When working weekends you are to take care of ALL clients and caregivers
· When working the weekend, 1 remote day in the week is included, however you must still be working at home (answering phones/texts, staffing, cleaning binders,etc.)
· 50% in the office and 50% in the field
· Duties also include calling for orientations, interviewing and scheduling qualified caregivers to come in.
· You may have to work a shift when needed if a replacement caregiver is unsuccessful however you MUST try to fill the shift with a caregiver FIRST before putting yourself in a shift.
