Major AreasofResponsibilities :

Patientand Family Educator : Educates the patient and family onthe disease process. Promotes and maintains patient health andindependence through teaching and appropriate rehabilitativemeasures while assisting patients in learning appropriate selfcaretechniques.

Advocates for patients and ensures participation incare planning and goals ofcare.

Plan of Care : Assists patients through managing their Plan of Care ando bserves and monitors their conditions and performsnecessary assessments.

Performs patient comprehensive assessmentsand collaborates with patient multidisciplinary care team (PT OT STMSW and paraprofessionals) to develop modify and implement anindividualized patient plan of care per physician orders thatensure quality treatment proper discharge planning and achievesdesired outcomes andgoals.

Documentation : Completes documentation timely and accurately includingOASIS assessments admissions recertifications subsequent visitsphysician orders and carecoordination.

Collaboration : Helps to achieve and maintain continuity of patient careby communicating information including changes in the plan of carenew orders lab results and referrals promptly.

Engages other careteam members when necessary to achieve desired patient outcomes.Facilitates interdisciplinary communication and followup throughcare coordination.

Participates in team meetings and caseconferences and leads discussions on multidisciplinary cases asappropriate. Maintains ongoing communication with a clinicalmanager regarding the progress of patients for assignedcaseload.

Requirements

RegisteredNurse (RN) JobRequirements :

• CurrentNurse licensure in the state ofTN
• Recent TB test or ChestXray
• Valid CPRCertification
• Perform dutiesoutlined in the clients plan ofcare
• Open patient case for LPN to doa followup
• Observe the patientphysical mental and emotionalconditions
• Document observed changesin patient s condition situation or careenvironment
• Maintain a professionalnature within patientshome
• Experience using Kinnser EMR(preferred)
