Shift : MondayFriday days with potential weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking a compassionate Hospice Home Health RN for a 13-week travel assignment in East Hartford, CT. The RN will provide skilled end-of-life care in patients homes while supporting families through the hospice process. Candidates must be experienced in hospice home health and proficient with Homecare Homebase.

Key Responsibilities :

Deliver compassionate hospice nursing care in the home setting

Monitor symptoms, provide comfort care, and update care plans as needed

Collaborate with the interdisciplinary hospice team to ensure continuity of care

Educate families and caregivers on symptom management and available resources

Document care thoroughly in Homecare Homebase

Qualifications :

Active RN license in Connecticut

Minimum 2 years of nursing experience, including at least 1 year in hospice home health

Proficiency with Homecare Homebase (required)

Excellent communication and clinical assessment skills

Ability to work independently and manage caseload efficiently

Reliable transportation, valid drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
