Job Description :

Sunbelt Staffing is hiring an experienced Hospice Home Health RN for a travel assignment in East Hartford, CT. This position offers the chance to provide meaningful end-of-life care in the home environment, ensuring patients comfort and dignity while supporting families. Homecare Homebase proficiency is required.

Key Responsibilities :

Conduct hospice nursing visits and provide direct patient care

Assess patient conditions and adjust care plans in collaboration with the care team

Coordinate with physicians, social workers, and hospice staff

Educate and counsel families and caregivers throughout the hospice process

Maintain timely and accurate documentation in Homecare Homebase

Qualifications :

Active Connecticut RN license

At least 2 years of nursing experience, including 1 year in hospice home health

Skilled in Homecare Homebase (required)

Strong interpersonal and clinical judgment skills

Ability to manage caseload independently

Drivers license, reliable vehicle, and auto insurance required

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
