Wells Health is hiring a Hybrid Care Manager Patient Care Technician to support seniors in Assisted Living Facility in New York.
This role is in-person
You will be the on-site Wells representative who welcomes residents, helps them get started with services, and checks in with them weekly to ensure they are doing well and staying connected to care.
Responsibilities
Meet new residents and introduce them to Wells Health
Collect consent from residents
Schedule and coordinate telehealth appointments
Assist residents during telehealth sessions when needed
Complete weekly wellness check-ins with every assigned resident
Track changes in mood, engagement, attendance, safety, or family involvement
Report clinical or behavioral concerns to Wells the same day
Maintain strong working relationships with facility staff
What success looks like
Reliable weekly check-ins with every assigned resident
Residents understand and attend their scheduled sessions
Issues are escalated quickly
Documentation is accurate and completed the same day
Requirements
Experience working with seniors in Skilled Nursing / Sub-Acute Rehab strongly preferred
PCT Certification/ CNA
CNA / social services / activities / admissions / case management background is useful
Strong communication and relationship-building skills
Highly organized and dependable — this role affects resident safety and support
Reliable transportation
Schedule
Part-time
Weekdays, on-site
