The Direct Care Professional (DCP) provides support, supervision, and assistance to individuals residing in a group home setting.
This role ensures that residents’ daily needs are met while promoting independence, dignity, and quality of life.
The DCP is responsible for implementing individualized care plans, supporting daily living activities, and fostering a safe, positive, and inclusive home environment.
Key Responsibilities Provide direct care and support in daily living activities, including personal hygiene, meal preparation, medication assistance, and transportation.
Support residents in achieving personal goals, skill development, and increased independence.
Ensure the health, safety, and well-being of all residents by following established policies, procedures, and care plans.
Assist in planning and facilitating recreational, educational, and social activities.
Document resident progress, incidents, and daily activities accurately and in a timely manner.
Administer and record medications in compliance with regulatory standards and agency policies.
Support positive behavior management and crisis intervention when necessary.
Maintain a clean, safe, and organized living environment.
Communicate effectively with residents, families, team members, and supervisors.
Participate in ongoing training and professional development.
