Job Title : Recovery Coach

Program(s) : Case Management

State : Tucson, Arizona

Population Served : Clinical

Reports To : Clinical Supervisor

FLSA Status : Non-Exempt

SUMMARY

The Recovery Coach coordinates and / or directly provides covered behavioral health services to address individual client needs. The Recovery Coach serves as a participating and facilitating member of the client’s team by providing services individually or as part of a team. The Recovery Coach is responsible for bridging services and gathering information from community resources.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Develop and implement individual service plans (ISP) and crisis plans.
  • Facilitate child and family team (CFT) meeting or the Adult Recovery Team (ART).
  • Assist clients in navigating the behavioral health system.
  • Aid the client in the identification and development of natural supports.
  • Provide support during crises by helping families develop coping strategies and finding immediate assistance.
  • Collaborate with mental health professionals to create crisis intervention plans tailored to the family’s situation.
  • Work collaboratively with family members, therapists, and other members of the care team to ensure comprehensive support.
  • Ensures that all services, procedures, and forms provided are culturally relevant, linguistically appropriate and gender responsive (including Lesbian, Gay, Bisexual, Transgender, and Questioning (LGBTQ).
  • Ensures that all clients are seen according to contract requirements.
  • Maintains accurate and confidential records of client interactions, progress, and outcomes and documents all records in the Electronic Health Record (EHR).
  • Completes the required number of job-related trainings according to all state and contractual required guidelines.
  • Ensures the safety, health, and welfare of staff and clients at all times.
  • Acts as a positive role model and mentor for children, parents, and their families.
  • Treats others with respect, confronts negative behavior, and supports confrontation.
  • Attends and participates in all required meetings including individual supervision and team meetings.

  • TRAVEL

  • 20% throughout county assigned.
  • REQUIRED EDUCATION AND / OR EXPERIENCE

  • Bachelor’s degree in Social Services, Psychology, or an unrelated Bachelor’s degree with one year of experience in the mental health field, or Master’s Degree in Social Services or related field without AZ state licensing
  • Ability to obtain and maintain a valid unrestricted Level One Fingerprint Clearance Card.
  • Minimum age requirement is 21.
  • Must meet the requirements of a safe driver.
  • Must possess a current Arizona State Driver’s License and have an acceptable driving record for the past three (5) years.
  • Must have personal vehicle liability insurance and vehicle registration as required.
  • When required, the ability to pass a criminal background clearance check, drug screen, and if requested a physical exam.
  • Must obtain and maintain CPR and First Aid certification upon hire.
  • Strong knowledge of the behavioral health systems processes, practice, and components.
  • Skills, knowledge, and ability to instruct clients in the following topics : child development, home management, personal problem solving, accessing community services, and supports domestic violence issues, appropriate discipline and childcare techniques and age-appropriate parenting skills.
  • PREFERRED EDUCATION AND EXPERIENCE

  • 2 years’ experience in social work or related field preferred.
  • Bilingual in Spanish preferred

Ability to write and maintain accurate and timely documentation. Ability to present information 1 : 1 and to groups in an organized and concise manner.

  • Must be able to work independently as well as a member of a team. Must have excellent organizational and time management skills. Must remain flexible and able to handle multiple priorities with the ability to adjust to high pressure and rapidly changing business conditions.

  • Ability to quickly assess unique situations and develop and apply strategies to effectively meet the needs of the child and family. Ability to interpret contracts and ensure compliance. Ability to provide exemplary service to employees and outside constituents.
