Job Title : OPWDD Self-Hire Staff

Status : Per Diem, Hourly

Reports To : Program Participant

Purpose : Any individual hired by a self-directed program participant is hired to complete a variety of habilitation and / or respite tasks as outlined in their habilitation plan. These tasks are determined by the participant, outlined in the plan, and are developed to be a support and / or meet a need that will allow the program participant to become more independent and achieve their valued outcomes as defined.

Essential Functions :

The goal of the Self-Hired staff is to fulfill the role as outlined in the Individualized Habilitation plan. Those roles can include but are not limited to any of the following :

• Skills development
• Housekeeping
• Community activities
• Community integration
• Job coaching
