Clinical Director – Home Care Services
Syracuse, NY
Salary Range : $85,000–$100,000, based on experience

NurseCore is seeking a Clinical Director to lead home care services in Syracuse, NY. This full-time role combines clinical expertise and leadership to ensure safe, high-quality patient care in compliance with New York state and federal regulations.
Responsibilities :
- Supervise nursing staff and Home Health Aides, ensuring training and competency
- Manage client care : assessments, care plans, recertifications, coordination, and discharge planning
- Oversee referrals, staffing, recruitment, onboarding, and scheduling
- Conduct home visits, clinical reviews, and PAG meetings
- Address client concerns and maintain accurate documentation
