Join Our Team as a Hospice Liaison

Are you skilled in coordination and relationship-building? Are you ready to make a significant impact in the lives of patients and their families? We are seeking dedicated hospice liaisons who are passionate about providing compassionate care and support.

As a hospice liaison, you will play a crucial role in coordinating patient referrals, building strong relationships with hospitals to secure these referrals, and educating the community about the importance of hospice care. Your efforts will ensure continuity of care, clarity in communication, and proper documentation as you collaborate closely with patients and their care teams.

Essential Functions

As a Hospice Liaison or Account Executive, you will coordinate and generate new business for hospice care by identifying new markets, maintaining existing accounts, and ensuring seamless care for referred patients. You will nurture relationships between the agency, hospitals, medical professionals, and other referral sources while educating them about our hospice services. This role requires daily interactions with patients, medical professionals, and community members to ensure effective communication and documentation, working closely with agency staff to deliver high-quality patient care.

Qualifications

  • A desire to serve patients and their families with compassion and dedication
  • Education : Minimum of an Associate Degree in a relevant field or equivalent experience in a health-related field
  • Experience : At least 2 years of sales experience in a clinical care setting, preferably in hospice
  • Required : Reliable transportation and the ability to sit, stand, bend, and lift at least 25lbs.
