Position Summary :

Serve as the food and nutrition expert who translates the science of nutrition into practical solutions for healthy living.

Work closely with TFHC staff at all levels using nutrition expertise to help patients make positive lifestyle changes, creating improved quality outcomes.

Integrate food and nutrition programs and services with patient experiences in primary care and behavioral health, coordinated with case management.

Essential Functions :

  • Advice and counsel patients and staff on food and nutrition.
  • Participate as member of interdisciplinary care team to explain nutrition issues to patients, assess their dietary and health needs, and help attain improved quality outcomes.

Work as needed in community settings, teaching, monitoring and advising the public and helping improve quality of life through healthy eating habits.

  • Educate patients by conducting and providing individual and group training about :
  • nutrition and administering medical nutrition therapy as part of the healthcare team.
  • the connection between food, fitness and health.
  • Educate patients and staff on food and nutrition issues and topics.
  • May suggest and / or develop meal plans and gauge the effects of these meal plans.
  • Promote nutrition through public speaking and community outreach programs, including food pantries and fresh markets.
  • Provide appropriate nutritional consulting services and communications for TFHC community events, public relations and marketing activities.
  • Keep abreast of the latest research in nutritional and food sciences.

Advocate for, and advance, individual and community nutritional status

General Duties :

  • Support the Mission, Vision and Values of TFHC
  • Other duties, as assigned.

Knowledge and Experience :

  • Two years or more of actively providing dietary counseling and education with attaining measured improvements in a community environment such as community health centers, primary care hospitals, rehabilitation centers and / or active adult communities.
  • Must demonstrate skills with relevant computer technology, including electronic health records

Education, Certification Training and License :

  • Holds and maintains current Registered Dietitian-RD or Registered Dietitian Nutritionist- RDN credential.
  • Professional specialty certifications are a plus.
  • Certification for TFHC value includes pediatric, oncology, gerontology, nutrition support and diabetes education.
