We are seeking  detail-driven  Document Imaging Techs / Medical Records Clerks  to support a Corporate HIM Operations. You’ll retrieve charts, prep / scan / index documents into the EMR, perform QA, and help ensure timely, accurate, and compliant medical records—work that directly supports patient care and revenue integrity.

Responsibilities

• Retrieve,  prep, scan , and  QA  patient records per HIM Prep / Scan Guidelines.
• Index  documents to the correct patient / encounter ( CSN ), document type, and location; split / merge, append / insert pages per UMMS indexing rules.
• Identify poor-quality images and  rescan  as needed; verify orientation / legibility.
• Work assigned  electronic worklists  daily and process items on time.
• Death certificate  processing : validate completion / submission per state regs; coordinate with physicians and funeral homes as needed.
• Provide  Release of Information  coverage as assigned.
• Meet and report weekly  productivity  and  quality  standards; assist with training new staff.
• Communicate issues promptly to the Team Lead / leadership.
