Primary Responsibilities :

• Serve the Southwestern Valley
• Complete senior wellness visits in clinics, virtual, and SNF / custodial settings to see 10-14 patients per day. May be asked to do home visits on occasion
• Travel to Optum outpatient clinics and independently contracted physician offices, as well as some home and SNF custodial visits, 1-2 locations per day
• Conduct comprehensive assessment, perform and discuss recommended screenings (such as QuantaFlo, Neurometrix, or point-of-care A1C), comprehensively document and code encounter, and conduct patient education and appropriate follow up
• Work a schedule of Monday - Friday, with occasional need for Saturday clinics

Required Qualifications :

Current California DEA certificate required prior to start date

Preferred Qualifications :

California Residents Only : The salary range for this role is $109,513 to $160,000 per year.
