About the Job :

Provides clinical social work interventions with oncology patients and families at the Elliot Regional cancer Center in Manchester and Londonderry that address personal and environmental issues to maximize emotional, social, and physical well-being and the appropriate and efficient use of health care services across the continuum.

Promotes efficient coordination of patient care through collaborative practice with the health care team. Promotes patient, family, staff and physician satisfaction through coordination and continuity of care.

Provides a systematic approach to evaluation of patient care. Coordinate’s patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care.

Provides ad-hoc support services and support groups to patients at the Elliot Breast Health Center.

What You’ll Do :

  • Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of each patient
  • Identifies relevant community resources and links them to patients as appropriate
  • Provides supportive counseling and crisis intervention for patient / family as needed
  • Coordinates with key personnel at all community health facilities, which treat oncology patients, to ensure patient needs are met
  • Coordinate’s homecare referrals, acute rehab, and nursing home referrals as defined
  • Serves, as a resource / liaison to community social service agencies relative to identified patient / family needs

Who You Are :

  • Master of Science in Social Work, Required
  • One (1) to three (3) years of experience in working with Oncology patients and cancer survivors, Required
  • Excellent clinical documentation, and successful usage of EMR systems, Preferred
