DEPT : HEALTH INFO MGMT SERVICES
Part-Time / 8 : 00am-4 : 30pm / 20 hours per week
Under general supervisor, preps, scans, and validates all types of paper documents into optical scanning system to obtain chart completion. Scans & validates all loose reports that are sent to the department. Responsible for analyzing medical records for deficiencies in chart documentation. Responsible for release of information for continuity of care requests. Assures that guidelines of HIPAA are followed to reduce the hospital's potential liability in releasing records or protected health information. Responsible for processing requests for government audits and specialized chart reviews (UVMC only). Responsible for monitoring incomplete charts and follow up with physicians. Responsible to analyze records to conduct regulatory documentation audits. Responsible for following guidelines of JC and Conditions of Participations and all federal and state laws. Responsible to perform other duties as assigned and needed.
Essential Duties & Functions
- Accurately and according to procedure guidelines preps, scans, and validates for all chart types.
- Accurately analyze charts checking for deficiencies in provider documentation : Acute Inpatients, Same Day Surgery, Emergency Department, Observation, and Behavior chart types following guidelines of JC and Conditions of Participations and all federal and state.
- Understand the HIPAA and Compliance Program.
- Perform Release of Information processes for continuity of care requests.
- Log all requests into the electronic medical record system (UVMC only).
- Process requests for attorneys, insurance companies, patients and / or their representatives in the requested delivery mode, government audits and specialized chart reviews (UVMC only).
- Responsible for picking up discharges.
- Assist with weekly monitoring and provider notification process based on Voluntary Relinquishment policy.
- Assist physicians and clinicians with chart completion.
- Assist with walk in customers in a timely and courteous manner.
- Responsible for monitoring Epic work queues, as assigned.
- Responsible for completing Regulatory Documentation Audits.
- Participate with validation testing during Epic upgrades, as needed.
- Maintain productivity and quality standards.
- Must have a good understanding of the Premier Medical Staff Bylaws / Rules & Regulations.

Provides general secretarial support.Update Excel spreadsheets.Performs other duties as assigned.Minimum Level of Education Required : High School completion / GED
Experience
Minimum Level of Experience Required : 1 - 3 years of job-related experience
Prior job title or occupational experience : Medical Office
Preferred experience : Health Information or related medical field, Knowledge of Word & Excel.
Other experience requirements : Medical Terminology or completion of medical terminology course within 3 months of hire.
Knowledge / Skills
Ability to read and write to review and maintain patient medical record charts, perform basic calculations, and operate standard office equipment.Ability to work independently in a medical office setting.Problem-solving abilities and basic analytical skills.Excellent organizational skills.Ability to type accurately at acceptable speeds.Interpersonal skills to communicate with patients, physicians, and staff via telephone or in person.Basic analytical skills to determine work priorities and improve workflow.Employment type : Full-time
