What is PACE?

PACE ( P rogram of A ll-Inclusive C are for the E lderly) was created to answer the many problems around caring for frail, and medically complex seniors. The PACE model is intended to keep individuals living safely and comfortably in their homes whenever possible.

HopeWest PACE participants receive wrap around services that include but are not limited to in-home care services, transportation to a HopeWest PACE center for primary medical care, social and recreational activities.

The Unique Responsibilities of the Associate Medical Director at HopeWest PACE :

• Serves as the clinical leader and resource for complex cases to internal and external clinicians.
• Provides direct, expert medical care to participants in a variety of settings such as clinic, day center urgent care, home, rehab, nursing home, and assisted living facilities.
• Actively collaborates with the entire PACE interdisciplinary team for the highest quality care of each participant.
• Assists with developing, planning, and providing in-service education for all PACE care team.
• Assists with the development of policies and procedures, establishing standards of care, and participating in quality assurance, compliance program, emergency procedures, as assigned and in conjunction with the PACE Medical Director.
• Participates in quality monitoring, quality improvement, and process improvement projects.
