Job Title : Medical Social Worker Profession : Social Worker Specialty : Social Work Duration : Direct Hire Shift : Days Hours per Shift : 24 hours per week Experience : Two years experience in an acute care hospital setting or related setting preferred License : Licensed Social Worker (LMSW / LCSW) approved by the State Education Department Certifications : NYS SCREEN Certification (obtained within initial 3 months of employment) Must-Have : Flu vaccine required, no exemptions Description : Under the general direction of the Director / Assistant Director / Manager of Pharmacy Services, the Registered Pharmacist provides indirect patient care. The Registered Pharmacist works with nurses, peers, physicians, and other members of the healthcare team in the delivery of pharmaceutical services to patients. The Registered Pharmacist reviews the patient’s medication orders against the patient’s medication profile in accordance with hospital and pharmacy philosophy, policies, procedures, and standards. The Registered Pharmacist demonstrates leadership and management skills and a commitment to customer service, professional accountability, and growth. Understands and adheres to the organization’s performance standards, policies, and behaviors. Completes a comprehensive initial assessment. Performs all activities for multidisciplinary care coordination at the intra-hospital and inter-hospital levels of care. Performs all transitional planning activities including, but not limited to, patient / caregiver education, arrangement of all aftercare, and commercial payer involvement. Communicates and collaborates all relevant patient information to appropriate health team members. Ensures department goals of length of stay reduction, readmission prevention, denial reduction, and satisfaction improvement are realized. Documents per department requirements.
