Schedule : Monday - Friday 8 : 30 AM - 5 : 00 PM with weekend rotation
Dept : Case Management - supporting trauma unit
Location : Presbyterian Medical Center - Charlotte, NC; Presbyterian is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte
This role is ideal for an experienced acute care RN looking to work in a patient facing but non-bedside role within a high-performing, collaborative team.

What We're Looking For :
Education : Associates Degree in Nursing or BSN, required. 4 Year / Bachelors Degree, preferred.Experience : Two or more years healthcare experience, preferred. Case Management or related experience, preferred.Licensure / Certification : Current RN licensure in state of employment, required.Additional skills required : Weekend and holiday coverage .Additional skills preferred : Knowledge / Competence of Microsoft office products and keyboarding.What You'll Do :
Complex discharge planningCollaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive / complex / safe discharge plansCollaborate with MSW case managers for guidance with pscyhosocially complex casesFacilitate coordination of careAccept responsibility, authority, and accountability for management of careServe as a resource for discharge planning and care coordination for medically complex casesMaintain current knowledge of available community resources / post acute care options
