The TCOOMMI CMH Juvenile Case Manager will provide Mental Health Case Management functions as outlined by the Texas Administrative Code and in accordance with the Texas Health and Human Service Commission Resiliency and Recovery addition this employee will adhere to the TCOOMMI contract requirements. Specifically this employee will work with a designated caseload of juveniles that have been referred from the local Community Supervision and Corrections Department. Mental Health recovery services assist an individual in gaining and coordinating access to necessary care and services appropriate to the individuals needs. Recovery Management functions include patient assistance treatment planning and monitoring of services crisis services and the provision of skills training. This employee will develop and maintain referral resources for service provision allowing for patient choice. Service focus is on the reduction of intensity and frequency of symptoms improvement in core community reintegration skills increase in self-reliance and improvement of independent living skills and community and family interactions. This position requires the development and maintenance of effective professional working relationships with community service partners and providers including but not limited to Juvenile SNDP Officers and other Juvenile Probation Officers.
The TCOOMMI CMH Juvenile Case Manager supports the mission and organizational goals of MHMR Services for the Concho Valley and Texas Department of Criminal Justice while developing new initiatives and strategic direction to provide the highest level of comprehensive behavioral healthcare for juveniles and their families who are in all phases of involvement in the Criminal Justice System. The TCOOMMI CMH Juvenile Case Manager will implement the principles of trauma-informed care including culture sensitivity in all areas of service delivery.
EDUCATION REQUIRED : Bachelors or advanced degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology social work medicine nursing rehabilitation counseling sociology human growth and development physician assistant gerontology special education educational psychology.
LICENSE / CERTIFICATION REQUIRED : Valid Texas drivers license required. Must be able to pass a NCIS and TCIS Criminal Background Check prior to employment. Must be able to pass an extensive background check to work with individuals involved in the legal system. Must be insurable on the Centers insurance. No more than 3 moving violations in the last 3 years must have proof of current liability insurance.
EXPERIENCE REQUIRED : Prefer one to two years experience in like or similar position either public or private. Education will be considered in lieu of experience.
KNOWLEDGE SKILLS AND ABILITIES REQUIRED : Must be capable of using Microsoft Office products including Excel Outlook and Word. Strong organizational skills. A demonstrated ability to work in a fast-paced environment. Ability to clearly communicate sensitive information both orally and in writing. Demonstrate the ability to work closely with all levels of service providers in an atmosphere of teamwork. Ability to manage multiple tasks simultaneously.
