Case Manager LVN (San Angelo)

Job Summary

Under the supervision of the Manager of Case Management and RN case managers, the Case Management LVN will collaborate with other members of the interdisciplinary team in developing and implementing patient discharge plans and utilization of hospital and post-acute services. This will include but is not limited to referrals, authorizations and a variety of clerical duties.
