Job description

The Cape & Islands Adult Community Clinical Services (ACCS) operated by the Department of Mental Health (DMH) is seeking a dynamic candidate who will provide direct care services to individuals in a residential group home setting. This position will act as a primary advocate for assigned Persons Served; assisting individuals to meet their rehabilitation goals, completing required daily documentation and maintaining a Person Centered, Trauma Informed approach.

The selected candidate will work as part of a residential group home team within the ACCS program and will provide rehabilitative interventions to persons with major mental illness by participating in the development of comprehensive assessments as well as developing and implementing the clients' Individual Action Plan. Additional responsibilities may include securing appropriate services for persons served by working with other programs within the agency as well as the community such as SSI, Mass rehab, day programs, psychiatric and medical services. Knowledge of all medications used within the program is expected as he or she will be distributing medications to clients and educating on proper usage. The incumbent in this position will maintain a therapeutic atmosphere through professional performance and attitude to promote quality care and person served growth. Transporting clients to and from appointments or activities using a program vehicle or their own vehicle is necessary for this role.

Please Note : The selected candidate will work 40 hours per week on the 2nd shift from 3 : 00 PM to 11 : 30 PM. The work location will be 191 Santuit-Newtown Road, Marstons Mills, MA 02648.

Position 1 : Days off will be Wednesday / Thursday.

Position 2 : Days off will be Friday / Saturday.

Employees in this position will be required to perform mandatory overtime.

Duties and Responsibilities (these duties and responsibilities are not all inclusive) :

  • Engage persons, individually and / or in group, in the development of meaningful activities, such as recreational, educational, health and wellness, within the program and the community to foster an improved quality of life.
  • Assist in the development and implementation of person served Individual Action Plans through direct observation of the person served, full knowledge of the person served history, and engagement with the person served in order to assist them in learning the necessary skills to live successfully in the community.
  • Provide rehabilitative interventions and utilize coping strategies, natural supports and safety planning to assist persons served in building and using skills and accessing resources to achieve treatment goals, manage symptoms and live productive and meaningful lives.
  • Knowledge of all medications used within the program, including their indications, risks and side effects, and the ability to teach person served about their medications and how to take them appropriately by utilizing MAP related material and training.
  • Transporting or arranging transportation for persons served that need that level of service.
  • Responsible for maintaining person served records in a timely manner by documenting in EMR and paper record as well as completing all other related forms to ensure compliance with DMH regulations and Rehab Options requirements.
  • Ongoing yard, house and vehicle maintenance and cleaning as assigned by supervisor or designee to ensure that the condition of GLE site / yard meets Rehab Option, and Licensing. Keep emergency exits maintained at all times.

Required Qualifications :

  • Ability to follow written and oral instructions.
  • Capacity to perform basic mathematics.
  • Proven ability to gather information through observing and questioning individuals.
  • Talent for communicating effectively in oral expression, writing clearly and concisely, and developing ideas in logical sequence.
  • Skilled in interacting with people who are under emotional stress with the ability to maintain a calm manner in stressful or emergency situations.
  • Special Requirements :

    Please note in accordance with an agreement between the Commonwealth of Massachusetts, Department of Mental Health and AFSCME / Council 93, employees assigned to a work unit where the Medication Administration Program (MAP) is used are required to pass MAP certification within 6 months of entering the position or 3 examination opportunities, whichever comes first. Employees are required to maintain MAP certification as long as you are working in said program.

    Possession of a current and valid class D Motor Vehicle Operator's license required for this function.

    Preferred Qualifications :

  • Experience in and / or knowledge of human services.
  • MAP Certification.
  • Experience with DMH regulations and program procedures.
  • Familiarity and implementation of MAP policies.
  • Experience working with individuals with serious, persistent mental illness, substance abuse and / or who have experienced homelessness.
  • Previous experience working in a residential setting.
  • Given the population served, bilingual or multilingual fluency.

Programs include a 16-bed acute in-patient unit, Case Management, Day Treatment Program, and Adult Community Clinical Services (ACCS). The CC&I CMHC is a Joint Commission certified facility.     Qualifications :

    MINIMUM ENTRANCE REQUIREMENTS : Applicants must have at least (A) one year of full-time or equivalent part-time experience in providing direct care service to the mentally ill, emotionally or physically handicapped, or (B) any equivalent combination of the required experience and the substitutions below.

    Substitutions :

    I. An Associate's degree or higher with a major in psychology, human services, mental health or retardation may be substituted for the required experience.

  • Education toward such a degree will be prorated on the basis of the proportion of the requirements actually completed.
  • Salary placement is determined by a combination of factors, including the candidate's years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division's Hiring Guidelines. For all bargaining unit positions (non-management), compensation is subject to the salary provisions outlined in the applicable collective bargaining agreement and will apply to placement within the appropriate salary range.
