Responsibilities
Reach out to patients following a recent hospital visit and connect them with a registered nurseObtain and document basic information about a patient's recent hospital stayReview and prioritize patient census for effective outreachExplain services and enroll patients into appropriate programs based on their needsAddress inquiries and schedule initial and annual appointments for patientsQualifications
High School diploma or equivalentMinimum of 1 year of experience in a call center, sales, healthcare, or customer-focused environmentAbility to work in a virtual setting with flexibility regarding time zones (MST and CST preferred)Strong communication skills to effectively engage with patientsProficient in entering documentation into designated systemsPreferred Qualifications
