Your Role as a Home Health Physical Therapist :
In this rewarding position, you will :
Deliver home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury.Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on each patient’s care plan and goals.Observe movements, conduct assessments, document progress, and adjust care plans as necessary.Assist patients with exercises and hands-on therapy to manage pain and enhance mobility.Educate patients about their care plans, established goals, exercises, and self-care techniques.Coach family members on the care plan, therapy goals, and safe exercise assistance.What We’re Looking For :
To thrive in this role, you should have :
A degree from an accredited Physical Therapist Program and an active PT license in Michigan.A minimum of 1 year of physical therapy experience, preferably in home healthcare within the last 5 years.Current CPR certification.Knowledge of state and federal home health regulations.Strong clinical judgment, excellent interpersonal skills, dexterity, and a compassionate approach.Why Choose Interim HealthCare?
