Case Manager 120 (Employment Specialist) - Perth Amboy, NJ

Salary - $42,000 per year

Responsibilities :

Works as part of a multi-disciplinary team grounded in the housing first approach, and coupled with an integrated service model to assist families in obtaining and retaining housing in Middlesex County

Responsible for establishing a trusting relationship with families and working with them to identify their vocational and / or employment goals

Use a holistic planning approach to assist families with life skills training, job readiness, skills building, and linkages to employment

Rotating 24 / 7 coverage required

Case management and case documentation

Case conferences with DCP&P and other providers, if necessary

Requirements : Bachelor’s Degree

Requires a flexible schedule including availability on nights

Valid driver’s license with an acceptable driving history
