Job Summary :
The Medical Social Worker plays a vital role in providing comprehensive psychosocial assessments and support to members, enhancing their ability to manage their own healthcare journey. Through effective assessments, interventions, educational programs, and connections to community resources, the Social Worker fosters respect for individuality and the diverse backgrounds of patients and families. This position contributes significantly to the medical team's understanding of patient beliefs, behaviors, and obstacles related to their health conditions.
Essential Responsibilities :
- Employing age-appropriate assessment and intervention techniques tailored to the needs of patients and families.
- Helping members create cost-effective healthcare plans by leveraging personal and community resources, as well as Kaiser Permanente programs.
- Promoting a patient-centered, multidisciplinary approach that addresses the personal, psychological, economic, and cultural needs of members.
- Maintaining an extensive knowledge of Kaiser Permanente services, community resources, and relevant laws affecting healthcare planning.
- Completing required documentation of services and Social Work care plans as per departmental policy.
- Engaging in program development, orientation, and educational activities that enhance members' abilities to direct their healthcare experiences.
Basic Qualifications :
Experience
At least one (1) year of experience in social work within a hospital, outpatient clinic, hospice, or long-term care setting, or completion of the Kaiser Permanente NW Social Worker Internship program.A minimum of three (3) additional years in a related environment.Education
Master's degree in social work from an accredited university.License, Certification, Registration
Credentials from primary work state required at hire; additional credentials from secondary states needed post-hire.Clinical Social Work Associate Certificate (Oregon) within 6 months of hire OR Licensed Clinical Social Worker (Oregon) within 6 months of hire OR Masters Social Worker License (Oregon) within 6 months of hire.Licensed Social Worker Associate-Independent Clinical (Washington) within 6 months of hire OR Certified Social Worker Associate License (Washington) within 6 months of hire OR Licensed Advanced Social Worker License (Washington) within 6 months of hire OR Licensed Independent Clinical Social Worker (Washington) within 6 months of hire.American Heart Association Basic Life Support certification within 3 months of hire.National Provider Identifier required at hire.Additional Requirements :
Demonstrated ability to collaborate effectively within a multidisciplinary team.Preferred Qualifications :
At least three (3) years of professional experience in medical social work settings.Possession of LCSW credentials.Familiarity with KPNW services and benefits.Expertise in short-term counseling strategies.Demonstrated leadership abilities within a multidisciplinary team.In-depth knowledge of both local and national healthcare resources.Notes :
